Sustanon 250 is a brand name that has become a generic term for a specific blend of four esterified testosterone compounds.

It was originally developed by Organon for hormone replacement therapy (TRT) with the intention of requiring less frequent injections (once a month in a clinical setting). However, for performance enhancement where stable blood levels are key, the differing half-lives make Sustanon a complex compound to manage.

The Blend (Per 250mg/ml)

A standard ampoule of Sustanon 250 contains:

  1. 30mg Testosterone Propionate (Short: ~19h half-life)
  2. 60mg Testosterone Phenylpropionate (Medium: ~4.5 days half-life)
  3. 60mg Testosterone Isocaproate (Medium: ~9 days half-life)
  4. 100mg Testosterone Decanoate (Long: ~15 days half-life)

Pharmacokinetics & Dosing

The logic behind the blend is to provide an immediate spike in testosterone (via Propionate) while the longer esters build up over time to provide a sustained release.

The Problem: Stability

While designed for infrequent TRT injections, for a bodybuilder, injecting Sustanon once a week results in a rollercoaster.

  • Days 1-2: The Propionate spikes levels high.
  • Days 3-7: The Propionate clears, causing a crash before the Decanoate fully takes over.

Dosing Strategy

To maintain stable levels and take advantage of the Propionate ester without crashing, Sustanon is best injected Every Other Day (EOD) or at minimum Every 3 Days (E3D).

Sustanon provides all the benefits of Testosterone:

  1. Muscle Mass & Strength: The primary driver of anabolism.
  2. Recovery: Enhanced protein synthesis and nitrogen retention.

Side Effects & Harm Reduction

Estrogen Management:

Estrogen Spikes: Because of the Propionate ester, Sustanon hits the system fast. This can cause a rapid conversion to estrogen. Users often find they need to dial in their AI (Aromatase Inhibitor) differently compared to a single ester like Enanthate.

1. Aromatization

Testosterone converts to Estrogen. High levels lead to water retention, fat gain, and gynecomastia.

  • Management: Bloodwork is key. Use an AI only if symptoms occur or bloodwork confirms high E2.

2. Androgenic Effects

Acne, oily skin, and hair loss (in those predisposed) are common due to conversion to DHT.

3. Injection Pain (PIP)

The Propionate ester in the blend can be irritating to the muscle tissue, causing more post-injection pain than Enanthate or Cypionate.

Tracking Sustanon

  4. PCT Timing: Because of the heavy Decanoate ester (100mg), Sustanon takes a long time to clear. You must wait 3-4 weeks after your last pin to start PCT, or the Decanoate will still be suppressing you.
